Below you will find lists of required documentation you must provide when applying for some of CAC’s programs and services.
Please note: This is not a full list of all programs and services which CAC offers. If you are in need of assistance you may always call the office and ask to speak to someone who can provide you with information about your specific issue.

List of documentation required to apply for case management (1st month’s rent assistance, court order of eviction, utility termination, etc.):
- Photo ID’s for everyone in the household 18 years and older
- Social Security cards for everyone in the household
- Verification of all household income for the last 30 days
- Court order of eviction or utility termination
- 1st month’s rent form filled out by the landlord
- Completed W-9 for landlord (for court order of eviction or 1st month’s rent)
- Additional documentation may be needed based on the situation
List of documentation required for energy assistance (home heating assistance, electric bill assistance, heat and/or electric termination, past due bill):
- Completed and signed OHEP application
- Photo ID’s for everyone in the household 18 years and older
- Social Security cards for everyone in the household
- Verification of all household income for the last 30 days
- Proof of residence
- Copy of most recent utility bill or termination notice
- Copy of most recent heating fuel bill or receipt if applicable
- Additional documentation may be needed based on the situation
List of documentation required to apply to receive monthly food from CAC’s food pantry:
- Your Photo ID
- At least one proof of the following eligibility document
- SNAP card
- Documents showing that you receive Medical Assistance
- TANF Documents
- Unemployment Documents
- Energy Assistance Recipient Documents
- Proof of income that show you are at or below 150% of the Federal Poverty Guideline
List of documentation required to apply for rental of CAC owned properties:
- Photo ID’s for everyone in the household 18 years and older
- Social Security cards for everyone in the household
- Verification of all household income for the last 30 days
- Completed rental application
- Completed income certification form
- Additional documentation may be needed based on the situation
